Online Questionnaire

Students complete the SDTLA by responding to an easy-to-understand questionnaire. The 153 questions are either true/false or multiple choice. In addition, students are asked 7 demographic questions at the beginning of the questionnaire and 15 questions related to their involvement in out-of-classroom activities.

Institutions may want students who participate in activities like a freshman orientation courses or student development workshops to complete the SDTLA for personal assessment purposes. Institutions can subscribe to the SDTLA for a certain number of administrations and can refer students to complete the inventory at their convenience. There is no need to preselect which students will have access to the instrument. SDTLA staff can limit access from computers only on your campus or even from a handful of designated machines.

Students use their email address and institutional-specific SDTLA password to gain access to the questionnaire immediately or complete it at a more convenient time. By signing in the students will enjoy the flexibility of completing the questionnaire in segments if they choose. If students begin the questionnaire without finishing, they can sign in at a later time and complete the instrument from where they left off. Upon completing the questionnaire students immediately are shown a report of their results. They can print off the report or return to view their results at any time by again signing in with their email addresses SDTLA passwords.

Institutions that use the SDTLA for instiutional assessment purposes in a pretest/postest schema will also have their students sign in. In this case the institution selects a sample of students and SDTLA staff send them emails inviting them to participate and providing them with SDTLA passwords. The students use these passwords along with their email addresses to sign into the site and complete the questionnaire. The same email addresses are used to contact the students to take the posttest.


Online Report

When students fully complete the online questionnaire they are immediately presented with a report that shows their scores on each of the subtasks and the Salubrious Lifestyle scale. Along with each score is a description of the subtask or scale plus a statement comparing the score to a norm group of the same gender and classification of the student.

Customization

Institutions may elect to have the questionnaire and the report webpages customized with elements from their websites to present students with a questionnaire and report that looks "home grown".
